DD Residential Group Home Services

Guardian Hands’ DD Residential Group Home Services offer a safe, 24/7 living environment for adults with developmental disabilities. Our community-based homes focus on dignity and personal growth rather than a facility feel.

Compassionate staff assist with daily care, medication, and life skills. By partnering with families and professionals, we ensure personalized, goal-oriented support. We empower residents to build meaningful routines and participate in community life, ensuring they feel respected, valued, and at home every day.

Community Integration

Community integration is core to the Guardian Hands philosophy. We believe individuals thrive through active participation, not isolation. Our services build meaningful connections via employment, volunteering, and social events tailored to personal goals.

Staff provide guidance, transportation, and encouragement to boost confidence in community settings. Beyond simple inclusion, we focus on real engagement—developing social skills and self-advocacy. By fostering these relationships, we help individuals discover personal interests and a true sense of belonging.

Supported Living Services

Our Supported and Supervised Living Services empower individuals to live independently in their own homes while receiving tailored assistance. We provide flexible support for budgeting, meal planning, and daily problem-solving, adjusting as needs evolve.

Our goal is to balance freedom with reliable guidance, emphasizing skill-building and confident decision-making. By fostering accountability and ownership of routines, we ensure residents remain active community members while maintaining the safety and stability necessary for long-term success.

Individualized Care Plans

Guardian Hands provides Individualized Care Plans tailored to each person’s unique needs and life goals. We reject "one-size-fits-all" solutions, instead developing plans collaboratively with families, guardians, and healthcare professionals. Each person-centered roadmap outlines specific goals for daily living, wellness, and personal development.

Regularly reviewed and adjusted, these plans ensure services remain effective and respect self-direction. This structured approach guarantees growth, accountability, and full alignment with state requirements and best practices.

Getting Started with Guardian Hands

  • The DD Waiver

The Developmental Disability (DD) Waiver is a Virginia state program that provides funding for services that support individuals with developmental disabilities in community-based settings. Rather than institutional care, the DD Waiver helps individuals receive residential support, supervised living services, and community integration while living in their communities.

Eligibility for the DD Waiver is determined through an assessment process coordinated by the local Community Services Board. Once approved, individuals may access services from licensed providers like Guardian Hands based on their unique needs and goals.

  • Community Services Board (CSB)

The Community Services Board (CSB) is the local agency responsible for evaluating eligibility for developmental disability services in Virginia. Each city or county has a CSB that serves as the single entry point for services, helping individuals and families understand their options and complete required assessments.

The CSB also assigns a Support Coordinator once eligibility is confirmed. This coordinator ensures services comply with state regulations and helps families navigate the process of obtaining a DD Waiver and selecting providers.

  • Support Coordinator

A Support Coordinator is a professional assigned through the CSB to help individuals and families navigate the complex service system. They act as essential advocates, assisting with the development of individualized service plans and initiating necessary referrals.

These coordinators work closely with providers like Guardian Hands to monitor care over time. Their primary goal is to ensure all services remain person-centered, effective, and perfectly aligned with the individual’s evolving life goals and preferences.
